Don't open with "is everything okay?" It invites a yes and closes the door. Open with what you have noticed and nothing else: "You have been quieter in our last few. I wanted to check in on that rather than run through the list." Then stop talking and let the silence do the work.

"I need to tell someone their work is not good enough. I have been putting it off."
The delay is the part doing damage, because every week you wait is a week they believe the work was fine. Name the gap against a specific expectation rather than against your impression of them, give one example, and agree what different looks like by a date. That is a performance conversation, not a verdict.

It is going to be weird for a while and pretending otherwise is what makes it worse. Say out loud that the relationship is changing, be clear about the things you can no longer be in the middle of, and then build the new kind of trust deliberately rather than hoping the old one carries over.
